Requesting bans and unbans

Just a quick note here, (sorry for intruding) but it would be very helpful for the officers and anyine that uses this sub-forum that you please follow the outline that Major Yamagata has posted. In particular the guidelines for the topic name.

Note the change in the Ban Request form.

It didn't make sense to put your name in the ban request title, so instead put the name of the player that would be receiving the ban. This makes for finding banned player's topics quicker.
